SINGLE SHEET SURVEYS
Utilizing the new Xerox 4850 Dual Color Laser Printer, printing of Scannable, single sheet questionnaires can
be generated directly in black and red ink on stock paper. VISA's specialists computer-compose the static
portions of the form (logos, headings, fixed questions and response bubbles). After customer FAX sign-off,
the composed questionnaire is transmitted to our printing house via computer modem.
A magnetic tape record for each scan page is generated. This record "calls in" the static portion of the form
and enters the variable portion (name/address, individual-specific questions, etc.). There are no costly printer
negatives and plates - everything is electronic!
Surveys are printed in 4,8,12,16 page or 20 page booklets in which the pages are pre-identified with booklet
number and page number in optically Scannable code and visual numbers. Each page automatically retains
its own identity at every stage of the processing and analysis cycle which ensures absolute accuracy.
The survey pages are scanned on both sides of the page simultaneously. Booklet pages are separated for
processing by guillotine cutting the booklets at the binding edge. The outside pages become the aligning edges
containing the scanner timing marks - again, ensuring accuracy in scanning.

VISA's Optical Mark Reader reads 5,500 sheets per hour throughput and with it's revolutionary ink read head,
marks made with ball point pen as well as pencil can easily be read.
EMPLOYEE SATISFACTION SURVEYS
By way of specific example, VISA composed a printed 140 questions into eight survey pages (four pages laser
duplex-printed and stapled) for a nationally known travel agency. Traditional manufacturing of 400 copies as
eight page booklets would have been cost and time prohibitive. VISA received the text of the survey on a
disk on Tuesday and delivered the certified, Scannable questionnaires to our client on Friday of the same
Responses were scanned and cross tabulations and pie charts generated for each question within one week of
receipt of the completed surveys.
